Serine/threonine kinase 4 (STK4), also known as MST1, is a cytoplasmic kinase structurally related to yeast Ste20p and is a central component of the Hippo signaling pathway, which is crucial for organ size control, tumor suppression, and regulation of cell proliferation and apoptosis. STK4 phosphorylates various substrates, including myelin basic protein, histone H2B, and FOXO transcription factors, and is activated by caspase cleavage during apoptosis, leading to chromatin condensation and DNA fragmentation. It also modulates cell polarity, migration, and epithelial-to-mesenchymal transition, with high STK4 expression linked to favorable outcomes in certain cancers. Pathogenic variants in STK4 cause an autosomal recessive immunodeficiency characterized by persistent viral, bacterial, and fungal infections, lymphopenia, autoimmune cytopenias, and impaired leukocyte migration and adhesion. Affected individuals often present with HPV-associated epidermodysplasia verruciformis, increased immunoglobulin G, and require targeted antimicrobial and immunosuppressive therapies.
