Neuronal pentraxin receptor (NPTXR) is a 65 kDa type II transmembrane glycoprotein predominantly expressed in the brain, especially in the hippocampus and cerebellum, where it acts as a central hub for the neuronal pentraxin family by binding NPTX1 and NPTX2 and anchoring them to the synaptic membrane. Structurally, NPTXR forms trimeric complexes and is involved in synaptic plasticity by mediating the clustering of AMPA glutamate receptors at excitatory synapses, thus regulating synaptic strength and remodeling. Functionally, it may also facilitate the uptake of synaptic material during synapse remodeling. Clinically, NPTXR has emerged as a promising cerebrospinal fluid (CSF) biomarker for neurodegenerative diseases, particularly Alzheimer's disease (AD), where its levels decrease progressively with disease severity and correlate with synaptic dysfunction and brain atrophy. This decline in NPTXR abundance distinguishes AD from other neurodegenerative conditions and may reflect the overall loss of cortical volume and synaptic integrity in affected individuals.
