Mesencephalic astrocyte derived neurotrophic factor (MANF) is a secreted, evolutionarily conserved protein with neuroprotective and anti-inflammatory properties. Structurally, MANF contains a saposin-like domain and an unstructured C-terminal region, enabling interactions with lipids and proteins like GRP78/BiP, a key chaperone in the endoplasmic reticulum (ER) stress response. MANF is expressed in various tissues, including the brain, pancreas, and immune cells, and functions as a stress-responsive cytoprotective agent by modulating ER homeostasis, reducing oxidative stress, and inhibiting apoptosis. It lacks classical neurotrophic factor receptor binding but exerts its effects through intracellular and extracellular mechanisms, including regulating the unfolded protein response (UPR) and inflammatory pathways. MANF deficiency is implicated in neurodegenerative disorders such as Parkinson’s and Alzheimer’s diseases, as well as diabetes, due to its role in pancreatic β-cell survival.
