Intercellular adhesion molecule 5 (ICAM5), also known as telencephalin, is a type I transmembrane glycoprotein belonging to the immunoglobulin superfamily, distinguished by its nine extracellular C2-type Ig-like domains, making it the largest member of the ICAM subgroup. ICAM5 is selectively expressed on the surface of telencephalic neurons in the central nervous system, where it plays a critical role in neuronal development, particularly in dendritic outgrowth, branching, and synapse formation. It mediates both homophilic adhesion between neurons and heterophilic interactions with leukocyte integrins, notably LFA-1, contributing to neuron-microglial communication and immune privilege within the brain. ICAM5’s unique structure and localization enable it to participate in neuronal signaling and cognitive processes. Disease associations include acute flaccid myelitis and acute hemorrhagic conjunctivitis, and genetic polymorphisms in ICAM5 have been linked to increased risk of breast cancer and possibly neurodegenerative conditions.
