Hydroxysteroid 11-beta dehydrogenase 1 (HSD11B1) is a microsomal enzyme that catalyzes the reversible conversion of cortisol to inactive cortisone, playing a crucial role in glucocorticoid metabolism. It is highly expressed in key metabolic tissues such as liver, adipose tissue, and the central nervous system. HSD11B1 is encoded by the HSD11B1 gene located on chromosome 1q32-q41 and consists of 292 amino acid residues with a molecular weight of 32.4 kDa. The enzyme belongs to the short-chain dehydrogenase family and requires NADPH as a cofactor. HSD11B1 has been implicated in various metabolic disorders, including obesity, insulin resistance, and type 2 diabetes. Increased HSD11B1 activity in adipose tissue has been associated with central obesity and metabolic syndrome. Additionally, recent studies have suggested a potential role for HSD11B1 in the pathogenesis of sarcopenia, with its expression negatively correlating with muscle mass in sarcopenic patients.
