HOP homeobox (HOPX) is a unique homeodomain protein that lacks the conserved residues required for direct DNA binding. Instead, it modulates gene expression through protein-protein interactions with transcriptional regulators such as serum response factor and histone deacetylases. HOPX is expressed in both embryonic and adult stem cells and is essential for the differentiation of various lineages, including cardiomyocytes, keratinocytes, and skeletal muscle. It plays a critical role in cardiac organogenesis. Clinically, HOPX functions as a tumor suppressor in many epithelial malignancies, where its promoter is frequently silenced by hypermethylation. Conversely, high HOPX expression in acute myeloid leukemia is associated with leukemia stem cell-like states and serves as an independent predictor of poor prognosis.
