HES4 (Hairy and Enhancer of Split 4) is a basic helix-loop-helix (bHLH) transcription factor encoded by a gene on human chromosome 1. It plays a key role in developmental processes, particularly in T-cell differentiation through Notch1 signaling, and in bone formation. Structurally, HES4 consists of 221 amino acids and features a bHLH domain critical for DNA binding and transcriptional regulation. Clinically, HES4 has been implicated in several diseases. In osteosarcoma, its overexpression correlates with aggressive tumor behavior and poor prognosis. Additionally, epigenetic dysregulation of HES4 is associated with Huntington's disease (HD), where hypermethylation of its promoter leads to altered neuronal gene expression and striatal degeneration. Furthermore, elevated HES4 expression in hepatocellular carcinoma (HCC) is linked to poor outcomes, highlighting its potential as a prognostic marker and therapeutic target.
