Gamma-glutamyltransferase 5 (GGT5) is a membrane-bound glycoprotein enzyme that plays a central role in glutathione homeostasis, xenobiotic metabolism, and leukotriene biosynthesis. Primarily expressed in the liver, kidney, and placenta, it catalyzes the transfer of the gamma-glutamyl group from glutathione to various amino acids and peptide acceptors. The enzyme consists of a heavy subunit and a catalytic light subunit, requiring divalent cations for optimal activity at neutral pH. Clinically, elevated serum levels of GGT5 serve as a sensitive biomarker for hepatobiliary disorders, including cholestasis, cirrhosis, and hepatocellular carcinoma, often appearing earlier than traditional markers like ALT or AST. Furthermore, GGT5 levels are associated with an increased risk of cardiovascular diseases, such as atherosclerosis and heart failure, as well as metabolic syndrome and chronic kidney disease. These associations reflect its involvement in oxidative stress and endothelial dysfunction, making it a valuable clinical indicator of systemic metabolic and hepatic health.
