Dipeptidase 1 (DPEP1) is a zinc-dependent metallodipeptidase and glycosylphosphatidylinositol-anchored membrane glycoprotein that forms disulfide-linked homodimers on the brush border of renal proximal tubules and other epithelia. It plays a central role in glutathione and leukotriene metabolism by hydrolyzing various dipeptides, notably converting leukotriene D4 to leukotriene E4. Additionally, DPEP1 acts as a neutrophil adhesion receptor, mediating leukocyte recruitment and inflammation in the kidneys, lungs, and liver. It is also the pharmacological target of cilastatin, which inhibits the hydrolysis of carbapenem antibiotics. Clinically, DPEP1 is significantly overexpressed in high-grade intraepithelial neoplasia and colorectal cancer, where it serves as an adverse prognostic marker and a potential diagnostic biomarker for tumor progression.
